Baseball Player Evaluation Waiver Required

Every player attending the baseball evaluations at Perfect Performance (Tysons Playground) MUST complete a mandatory online waiver prior to arrival at the facility.  Please complete the waiver ASAP.  

Please disregard the language regarding memberships, packages and camps and clinics.  The only thing that applies to you for this engagement is the waiver of liability.
